The Ministry of Labour, Immigration, Training and Skills Development has issued two safety compliance requirements to Matthews Equipment Limited/Herc Rentals following a fatal workplace accident at Ontario Place on September 17, 2024. The tragic incident occurred when a piece of equipment flipped over, resulting in the worker’s death. Two ministry inspectors were sent to the scene to investigate the accident.

In an update, the Ministry stated: “A worker sustained fatal injuries as a result of equipment flipping over. The employer, Matthews Equipment Limited/Herc Rentals, was issued two requirements. Further details are not available at this time as the investigation is ongoing.”

This fatal incident took place during the redevelopment of Ontario Place, though it remains unclear whether the equipment involved was part of the larger construction effort. Early reports suggest the worker was involved in a mishap with a lifting device, which led to crushing injuries. Emergency services responded at approximately 9:30 a.m., but the worker was pronounced dead at the scene​.

The Ministry’s initial response to the tragedy emphasized its commitment to conducting a thorough investigation, and expressed remorse.  "We extend our deepest condolences to the family, friends, and colleagues of the worker during this difficult time,” reads the statement.

The redevelopment project includes upgrades to critical infrastructure, such as sewage, water, electrical and gas services, and included the use of heavy machinery and construction equipment.

This investigation will likely focus on equipment safety protocols and whether the necessary precautions were in place at the worksite. Matthews Equipment Limited/Herc Rentals will need to meet the compliance requirements issued, as investigators assess the cause and investigate possible regulatory violations.
